UTS has developed the following forms to assist in preparing and lodging access applications or requesting an internal review under the Government Information (Public Access) Act 2009 (NSW). Use of these forms is optional but preferred.
- Online access application form
This form includes a secure payment gateway for credit card payments. It can also be used if paying the application fee by cheque or money order. Simply follow the instructions provided. - Manual access application form (PDF 770kB)
This form can be used if you do not wish to use the online application form. The PDF can be edited in Adobe Acrobat and saved or printed. Refer to making a payment below for information on how to pay the application fee. - Request for an internal review (PDF 806kB)
This form can be used to request UTS review a decision made under an access application. The PDF can be filled out in your web browser and saved or printed. Refer to making a payment below for information on how to pay the application fee.

Credit card payments
Credit card numbers are not to be provided on your application form or emailed to the university. If you are paying the application fee using the online application form, you will be provided a link directly to the secure payment gateway. For other application fees or processing charges, use the payment link provided below. Ensure you enter the correct amount and send payment confirmation to right.to.information@uts.edu.au
