To receive support from the Accessibility Service, you will need to register with us.
Register with the Accessibility Service
Registering with Accessibility Service gives you the chance to discuss your learning, assessment and related requirements and plan any support you might need. As some of the supports and services we can provide need early planning, we encourage you to register as soon as possible e.g. as soon as you receive an offer.
How to register
There are four steps to complete your registration with the Accessibility Service:
- Book an appointment
- Complete the UTS Accessibility Service Registration form
- Provide supporting documentation
- Confirm your Access Plan.
1. Book an appointment
Book your appointment with an Accessibility Consultant (AC).
2. Complete the registration form
Fill out the UTS Accessibility Service Registration form before your appointment.
The form will take approximately 15 minutes to complete, but you can choose to fill out only the mandatory questions if you prefer.
This helps your Accessibility Consultant prepare for your appointment.
3. Provide supporting documentation
Bring documentation from a registered health professional to your appointment. They can either:
- Complete the UTS Accessibility Service Health Professional Document Form (PDF, 143kB) OR
- Write a letter on official letterhead that includes the necessary information outlined in the UTS Accessibility Service Documentation Guidelines (PDF, 64kB). The letter must be dated and signed.
If you don’t have documentation and need help obtaining an assessment, diagnosis, or finding previous records, feel free to discuss this further during your appointment.
4. Confirm Your Access Plan
In your first appointment, you’ll confirm your Access Plan and discuss your specific needs and any questions you have.
What to expect in your appointment
- You will meet your assigned Accessibility Consultant (AC); your point of contact for Accessibility Services.
- Your AC will explain the available services and listen to your priorities.
- The focus will be on what support you need for your studies and assessments.
- You can discuss how often you want to meet in the future (e.g., regular appointments or as needed).
- Your AC will provide information about available services and assistance.
