RFT2010/11-05 Professional Integration
Australian Government Department of Health and Ageing ($1,200,000 2011-2013) Tender awarded to: Price Waterhouse Coopers, University of Technology, Sydney School of Pharmacy, Australian General Practice Network.
This project aims to identify a best practice model for the integration of community pharmacists in the primary health care setting. The outcome will be a set of recommendations and implementation requirements for best practice models for professional integration. In particular, the roles or interventions community pharmacists should engage in and the mechanisms and enablers to achieving change. This endeavour is timely and important given the current context of primary care reform (aimed at improving access, prevention and coordination of care) and the evidence that has already been collected on the benefits of multidisciplinary care.
