How to fill out your Change of Student Details application
This form can be used to request updates to your personal details in My Student Admin.
You will need to provide certified copies of your supporting documents to make changes or corrections to your name and date of birth.
This means an authorised officer must certify a copy of a document as a true copy of your original document. We accept documents that have been certified at the UTS Student Centre in Building 10. Read more about Providing certified documents to UTS.
Your formal name at UTS should reflect your full legal name.
This means that your full legal name will be displayed on your graduation documents: First Given Name, Other Given Names, Family Name. You are also able to nominate a preferred name which can be used throughout your studies, but will not appear on your documents.
If you are updating your name or date of birth:
Please check that your personal information at the Australian Taxation Office (ATO) and the Unique Student Identifier (USI) Registry Office is the same as the information at UTS. This will avoid the risk of having an unverified USI or TFN which will have an impact on your graduation. Read more about ensuring that your USI can be verified.
If you are a graduating student:
If you are graduating, and you want your new name to reflect on your graduation documents, you must submit this form before the last day to submit a formal name change before conferral which can be found in the UTS principal dates. Graduating students who submit a change of student details application after the deadline will be contacted, as fees will apply.

Information provided on this form will be used to correct details held by the Lifetime Learner Experience Unit (LLEU) and information held in other systems, which are integrated with, or drawn from, LLEU. Information will only be disclosed where required by law, or where consent has been provided by the individual to inform others of the required changes. If your request to correct information has been refused, you will be informed. If you object to the reason provided, contact the UTS Privacy Officer.
